Discharge planning tends to assume a household that can absorb it. The first fortnight home is when medications get confused, when the follow-up appointment gets missed, and when a person who was managing before suddenly is not.
If you know a discharge is coming, tell us then, so the house can be ready before they arrive: bed downstairs, bathroom cleared, walker route checked, food in.
Someone comes home with a discharge summary listing medications that do not match the ones in the cupboard. Some were stopped in hospital. Some were changed. Nobody threw the old ones away.
Our caregivers do not change or sort medication; that is for the prescriber or the pharmacist. But when something does not match, they flag it to our office, and we let you know.
The goal is to need us less. We review the plan as recovery progresses and recommend fewer hours when the person is ready.
